There's got to be a typo here.
Does the Unspooled Vestige actually need to make a save if it successfully attacks, and actually takes damage if it fails...?
It has to be the creature it is entangling that needs to save, right?

If the filament attack hits, then both entanglement and unspool happen. Unspool does not affect the PCs; it affects the attacking unspooled vestige. The unspooled vestige has to make a saving throw or take damage.
The effect of Unspool is separate from and has nothing to do with a target affected by entanglement, other than that they both trigger at the same time from a successful attack with the filament.
The creature's description before the stat block says
Note that "Slip Out of Reality" also affects the unspooled vestige.
