Once the issue of Carman Rajani has been resolved, any interactions with the people of Otari seem to drop away.

Is this a problem for most people?

If so, what side quests could be added to keep Otari relevant?

I suppose it really depends on how much you expanded the story?

In the campaign I'm running, our cleric grew up and orphan raised in the Dawnflower Library, so they've been running side quest eliminating Lamashtan cults and getting bonuses for prominent undead slain.
Our occultist poppet is trying to figure out how to help a child who

Trigger warning child harm:
cultist's parents had used them as test subject for flesh warping
, so they've spent a lot of time going between Morlibent to ask for books they learn about to try an undo what was done to them.
And our pirate rouge, well.. she can't stop hitting on Yinyismara so..

Overall it really just depends on how you develop out their connections to the town.

The adventure stops presenting Otari content at about this point, yes. It seems to be assumed that the PC's will spend their resting time in Yldaris once they start exploring floors 7-10.
