1) Some way to handle crafted items - for example, currently my Alchemists have adapted one of the "Wand" sheets (that feature just the Wand lines - something like a dozen on the sheet - these are not official, but work great for a PC with lots of wands). I'll note the item, cost per unit of 3 (to craft) and as I make them the upper line of small boxes (usually checked off as the wand is used) is filled in with the CR number that I crafted them on, and the lower line of boxes is filled in as I use them. But those boxes are REALLY SMALL!
2) More lines - for more items bought. There are never enough lines... and after two sheets it get's REALLY hard to track items.
3) Some way to produce an Updated List of what you currently have. Items can easily get lost... and we need to skip lines after each item that can be upgraded, as an "Upgrade Mithril Chain Armor to (+1) can get lost. I found one player who had upgraded his Cloak of Resistance 3 times... each time to (+2). That's ok, he also bought it twice. On the same sheet.
These are a few just on the spur of the moment, I'm sure other people can point out other problems....
Bonus) Don't get me started on the problem of tracking the items twice - once on the CR and again on the ITS. Add in the fact that not everything is listed on the ITS.... grrr.
