Thank you for letting us know about this! I'm sorry to hear that you're missing a card, but we'll be glad to get a replacement set up to be sent out to you. Before I do that, I just wanted to verify that we have the correct shipping address for you. It looks like we only have one on file, but I just wanted to make sure it's the right one. You should be able to see the address if you go to the My Addresses section of your account. If what you see there is incorrect, please send your preferred mailing address to customer.service@paizo.com, and we can get it entered into the system.
