Thank you for pointing this out to us! It looks like canceling the subscription in advance made it grumpy, so it was withholding that last book from you. I've got it all set up in an order to be sent out to you soon, and you'll get the PDF as soon as it ships.
As for the Crimson Throne hardcover, it only comes with the PDF if you have an active Adventure Path subscription and have preordered the book. I'm afraid that since your AP subscription was canceled, you would not get the PDF unless you were to restart the subscription. I apologize for any confusion of inconvenience that this may have caused.

Unfortunately, the subscription does have to be currently active in order to get the PDF with the book, and you have to have received the last volume that was released. I'm very sorry for any confusion or inconvenience that this may have caused.
