These house rules will allow players to benefit from the effects of an "extended rest" HP restoration more than once per day, as I found that the extended rest rules never seem to heal enough HP.
Players may Heal HP based on the meal they eat up to a maximum of 3 times a day, the amount healed depends on the survival check of the "chef." Rations of any kind are exempt from this rule as they haven't been "cooked recently."

- Terrible Meal: Everything in the meal is either burnt, under cooked or not cooked at all. It tastes awful, and heals no HP.
- Lesser Meal: Most of this meal is under cooked, some of it is raw, and bits and pieces are burnt. It isn't good, but it is tolerable. This meal heals 1 HP every 4 HD you posses.
- Decent Meal: The meal is cooked to standard, nothing's perfect, but at least nothing's burnt. It tastes as well as something you'd get from a random inn or tavern. It heals 1 HP every 2 HD you posses.
- Excellent Meal: This meal is cooked extremely well. It's like you're eating at a fine banquet in a noble's manor. It heals 1 HP for every HD you possess.
- Perfect Meal: You're in heaven. This meal is cooked to perfection, everything tastes better than anything you could have imagined. This meal heals 2 HP for every HD you possess.

Obviously, how your group runs a game is your own business, but healing from meals three times a day seems like too much to me. However, I would agree that healing will happen faster when your wounds are tended to, you have a safe and comfortable place to stay, and you eat well enough as to not becomes malnourished. I suppose I might instead say that eating healthy meals (not rations) and resting would grant a bonus to Heal checks made of you.

Do this as profession chef, or craft meal and it is limited enough in usability to actually be balanced as a skill which can be considered and maybe chosen.
This, but I would actually say increase the bump but make it Temp HP. That way it's not the same as healing, can run out after a certain time, and can't stack with itself. That way it fills a new niche in healing rather than competeing in the same space as channel, treat deadly wounds, or clw potions.
Other thought is to maybe make the points die related so like instead of 1/4 hd, 1/2, 1 hd go like 1d3, 1d4, 1d6, etc. This helps you avoid being able to get back 40 free hp at 20th without having to expend expensive resources.
Also make sure to put a long time on the action like 10-30 mins per check. Helps avoid a weird moment where the wizard miraculously prepares a gourmet meal and throws out a fireball in 6 seconds at 5th level.

I like the idea but I think the execution needs a little work. I'm guessing some of the basis for this house rule are sayings like 'a good breakfast is the most important meal of the day' and 'an army marches on its stomach'. I also think, having read your terrible meal, that you could get food poisoning from a truly bad meal. Also a hearty, nourishing meal is not necessarily the same as a delicious banquet.
To me the skill is Craft(Cooking).
0 or less: Food Poisoning, 2 Fortitude saves required (DC10+negative amount) First to negate Nauseated condition, second to negate sickened condition. One save per 24 hours.
1-4: Terrible Meal, upset stomach, fortitude(DC10) save or sickened for 24 hours. No healing whilst resting
5-9: Poor Meal, No healing whilst resting for next 4 hours
10-14: Decent Meal no in game effects.
15-19: Hearty Meal, healing rates double whilst resting for next 1/2 day
25+: Excellent Meal, healing rates double whilst resting, +2 CON bonus for next 1/2 day.
A Take 10 is usually available, unless the PC is in a cooking competition or has to impress a noble etc.
