I figure that the mainline core gods view the AO pantheon equal to the minor deities and empyreal lords. Wati and Tephu both have a handful of high level priests of the old gods, so at least there they are a more prevalent religious institution than the lesser gods.
The old Kelesh sarenite faith has really fallen out of fashion in this far west region, with neither city's npcs entry having a sarenite in them. Pharasma is the big one in Wati, and Nethys in Tephu, and neither of them really care about other local cults if they're not one directly opposed to theirs (ie undead or antimagic).
I figure the everyman would see the old God worship a bit like mainline religions view old fringe ones today: skepticism, a bit of mistrust, and pointing out the arcaneness of their old ways.

Forgive me for necro-ing an old thread, but I've found that incorporating a priest of Osiris to work as a go-between for the Hall of Blessed Rebirth and the Grand Mausoleum is a really flavourful way to give worshippers of the Old Gods a point of connection, and demonstrate how the more traditional priests are trying to make a comeback but are still overshadowed by Pharasma, Sarenrae, Nethys, etc. It also highlighted some of the political tensions of the city - Sebti and Bahjut notoriously don't get along. It also gave the party someone to talk to for information about the city, a bit more accessible and less busy than Ptemenib. I even considered replacing Ptemenib entirely, but decided against it.
