I have a question regarding the card setup once you reach deck 4 and beyond of any set, in our case Island of Empty Eyes. I thought I had read that upon reaching deck 4 all cards of the current deck number (-3) are removed from play. So in my case since we have reached deck 4 this would have us remove all deck 1 cards from play that are still in the box. Is this correct?
or, do you simply just keep adding each new deck until you reach the end with each boon and bane containing all cards from decks 1-6?

That is not correct. You add cards to the box as normal, card removal is as specified on the Adventure Path card, namely:
After you begin Deck 3, when you would banish a bane that has the Basic trait, remove it from the game; when you would banish a boon that has the Basic trait, you may remove it from the game.
After you begin Deck 5, do the same for cards that have the Elite trait.
You never outright remove cards from the box as you add new sets in unless you are playing by Organized Play rules, which are slightly different than what is specified on the Adventure Path card. Instead, cards are removed as they are banished; note that you must remove banes with the Basic/Elite trait as you banish them in the appropriate adventures, but removing boons with those traits is optional.

That's a modification of the rule for Guild play, which has you removing all cards with the Basic and Elite traits whose number is (adventure - 3).
The Guild rule is a simplification of the rule on the Adventure Path card for your set. It should let you, after starting Adventure 3, remove boons/banes with the Basic trait from the game when the card would normally be banished. Elites can go after starting Adventure 4.
Otherwise yes, you keep adding cards to your set.
