I've recently updated this again, and try to keep it up to date as much as possible (please keep in mind I'm in law school, so it may be a few days). This was based off a sheet someone had given me at a convention which is by far out of date over a year later.
Instead of being something to slip into a folder, we use this to track the players of the local group as we've run into issues of what can people play without replay. It's open to anyone to view, I just ask if you want to use it make a copy of it to edit.
If anyone else has similar things that are willing to share please feel free to add. If you have any requests for similar things, I would be willing to hear them out and possibly able to make them over time.
