I have read the guide and was unable to find these answers. I might have just missed them.
I am considering some of the high lvl modules and was wondering a couple of items:
1) Do you need special permission to run a scenario above level 12?
2) How do you get the levels needed to continue the track? Rise of the Runelords skips levels and due to the tiers it does not allow you to run straight through from lvl 14 to the next lvl which is lvl 16.

1) Nope, there's no restrictions
2) You'll have to run something level-appropriate in-between. So if your RotRL takes you to level 15, and you need lvl 16 for the next book, run a module that is appropriate for level 15 to raise your group a level.
I'm doing this with a group on the board right now, taking them all the way to 18 (!)

Also, and this might be relevant, there are two ways to run the various APs.
Way 1: Run just the PFS sanctioned portions of the AP, but that will leave gaps between the segments, which can be filled by various sanctioned modules, or sanctioned segments form other AP.
Way 2: Run the entire AP in campaign mode, using non-PFS PCs, just assigning the PFS Chronicles earned to one (or more) of your PFS PCs as they are earned.
Campaign mode means that the PCs used do not need to be PFS legal, nor do you even have to use the Pathfinder game system, you could run it using a different game system, as long as the story remains true to the AP.
For example, I am currently running the AP Rise of the Runelords in campaign mode, but allowing only CRB options, traits from the Web document, and anything else requires GM approval.
