Need info on specific issue

Hello, I'm kind of new here.

I need some information on something that appeared in a specific issue of Dungeon. I even know the exact issue and module: #72, "Deep Trouble in Telthin"

Unfortunately, info on this issue is VERY hard to find. (All I can really find online is the cover art.)

Specifically, I need info on a unique magical item that appeared in that module. If anyone could help here, I'd appreciate it.

It is a mirror of truth. Any location or being in the known planes can be viewed with a chance of success equal to that of a teleport spell. the user can also conjure the image of any being and ask it a series of questions, one question for every 5 levels of the user. The being in the image immediately knows the identify of the mirror's user. If the being knows the answer (determined by DM) the being must answer truthfully.

Once per month the mirror can rain destruction upon a location viewed by calling an acid storm, earthquake, or storm of vengeance with double normal duration, after which the user's STR is reduced to 3 and he must rest for 1 week.

Any use of the mirror has a 20% chance of causing insanity (reduced by 5% per INT point over 15).

There is quite a bit more fluff and some mechanics, but that is the gist of it.
