Since it has no blessing timer, is it possible to simple lose into the eye without dying? Is there any way to back out if things go poorly, or is this a "come back with your shield or on it" moment?

One mitigating factor, though, is that, because there is no time limit, you can spend as many turns as you want positioning, resetting your hand for better cards, healing, etc.

That gives you a bit of a time limit.
Yeah, the important part is the Runewell location deck. At the start of each turn, you're burying a card. The more time you spend setting up, the more you're hurting yourself, until eventually, you're out of cards and die.
