I've gone over the heritages in blood of fiends a bit and found that they're quite interesting (more so than just getting the stats I want).
I have a few questions about them though.
1) If I choose a heritage do I gain the racial traits at the bottom of each of the entries, in addition to the alternate stats/spell-like/skills? EDIT: thanks for the answer, thought it was a racial trait, not a race trait.
2) If I do gain the race traits from the the heritage, do I gain them for free or do I have to replace current tiefling race traits with them, at one heritage race trait replacing one tiefling race trait? EDIT: again, confusion of race v racial.
3) Am I able to choose alternate racial traits that replace the spell-like or the skilled traits EDIT: if I have a heritage which gains an alternate spell-like and different skill bonuses (forgot to add that part in the first time)?
4) This is just a flavour/appearance question, but can (or even must) a rakshasa-spawn (beastbrood) tiefling have reversed palms/hands like rakshasa do?
These questions may have been answered already before, but my google-fu/search-fu have failed me if it be true.
I know this should be asked in the rules board, but I'd like to get a strict PFS ruling, rather than the usual "up to GM discretion" answer.

First off seems like you are confusion Racial Traits with Race Traits. What at the bottom of each Heritage page is a Race Trait not a Racial Trait.
As described in the Advanced Players Guide pg 8
RACIAL TRAITS
It is important to note that these racial traits are not the same as the trait rules found in Chapter 8 of this book (and originating in other Pathfinder products). Those traits are effectively half feats, intended to tie characters to the specific nations, cultures, regions, and races of the Golarion campaign world. Racial traits, on the other hand, are those racial abilities described at the bottom of each race’s descriptive page in Chapter 2 of the Pathfinder RPG Core Rulebook.
Each Race has Racial traits, there are books that allow you to swab out one Racial Trait for another.
Traits on the other hand are an optional rule first described in Advanced Players Guide Ch 8 that PFS uses, you can only pick 2 as a starting character and only 1 from each category, Race Trait is a Category.
1. No you can choose 1 of them as the 2 Traits you get as a starting PC. As always character can have only one trait from each category and these are considered race traits category.
2. Question is not relevant since you are confused between Racial Traits with Race Traits. Any option that allows you to pick a different racial trait will describe what trait it replaces. Since these are Race Traits the question is not accurate.
3. There are Racial Traits that replace those, these are not them, you will want to look in the Advanced Race Guide for optional Racial traits for Tiefling.
4. I cannot answer that question beyond my scope. But I would not recommend adding any physical changes not already describe in the text of the Rakshasa-Spawn.

There are Combat, Faction, Faith, Religion, Magic, Regional, Equipment, Race, and Social traits. Traits are akin to "half a feat", and all characters begin play with two traits for free. The Blood of Fiends book added more Race traits to the list that Tieflings could choose from. After you pick you heritage, you may choose one of those two traits listed at the bottom as your Race trait, if you so desire. If you do, you may pick one other trait from one of the other categories I listed earlier.
The confusion between "Race traits" and "Racial traits" gets a lot of people. Hope these explanations help.
