does the person use the stats for it in the Bestiary 3 exactly as is, modified by the druid companion table?

You'd use the animal companion stats, modified by the druid animal companion table. The animal companion stats are in the back of the Bestiary 3 (unlike the other bestiaries which include them on the same page as the creature itself) in an appendix.
