Rules for Exploring a Hex

I was wondering if anyone could point me in the right direction or tell me where the rules are for entering/exploring a hex. For example. The PCs enter an unexplored hex. What do I roll for or against? Do I tell them if something is in the Hex or do they have to find everything on there own? How often random encounters happen? Any help at all, even if it's just a house rule would really be appreciated.

Wandering Monsters: 5% chance when the players enter a hex, 15% chance per day

Landmark Sites: automatically discovered when they enter a hex
Standard Sites: automatically discovered after hex is fully explored
Hidden Site: skill check varies by site, listed in each location, rolled after hex is fully explored.
