GM Award question and tiers - New GM

I just ran my first scenario and have a question about GM award. The GM gets to apply the max prestige award and 1 XP to their character. Do I add my character to the reporting sheet? Also, My character is a level 4, does he get the level 4-5 reward even though it was run at 1-2?
