How do you know when a game is DEAD!

I did not play Dragonlance in the War of the Lance era but I would love to do so. I think the solution is more rest in between encounters and perhaps allowing a heal check to heal 1d4 points of damage per day if not per wound (Dark Heresy allows a heal check for each wound taken). A somewhat powerful house rule but one that allows for a steady campaign with no clerics I think.
