Good deal. I was prepping for a preview Pathfinder game at our local meet-up tonight and wanted to make sure I had the pre-gens as "right" as possible.
So CMD is equal to 10 + BAB + Str modifier + Dex modifier + special size modifier + miscellaneous AC bonuses (such as circumstance, cover, dodge, and deflection).
Another question, I've seen this stated another way as CMD = 10 + CMB + Dex modifier + misc AC bonuses. That would collapse the BAB, Str mod, and size mod into one additive that is already calculated, so that's good. For Tiny and smaller creatures, however, would they truly add their Dex mod twice (since they don't use Str mod for CMB)?

No, they wouldn't--which is why that formula for determining CMD is incorrect (probably based on people trying to reverse-engineer it from the iconic previews).
