I think I'm missing something terribly obvious on the heavy warhorse. It lists the BAB as +3 with a Str mod of +4 so why does the hoof attack at +6? I'm thinking it's because the horse has multiple natural attacks but then I think the penalties would be much higher (at least -2, making it +5 instead of +6).
Anybody have the (likely easy) answer to this question?

Fleece is right, the size penalty of -1. And the multiple natural attack penalty would only apply to its secondary attacks, not its primary one(s) (the hooves).

Size Mod is -1.
Oh mang! I feel so slow for not realising that earlier! Thanks a lot.
Hey I'm still tryin to figure out why Shadowmist, the horse found in RoTRL is dealing out more but whoopin than the partys fighter and were in Skinsaw Murders.... I actually feel sorry for the monsters at the farm.
Haha, it's clearly because horses are superior to humanoids in every way. They merely let the feeble "two-leggers" think they are stronger to keep the peace. If only we really understood the beastliness of the horse!
