I have this player who has reached a high enough level (both in XP and acomplishment) to deserve a promotion. What I don't know is:
1) Who? - Iomedae's temples are only inhabited by trainees, and the infirm or elderly, so who's going to promote this cleric? Where to find a Sword Knight around Magnimar?
2) How? - Gods and Magic doesn't mention a "capital" for Iomedae's cult, so even if I promote her, what makes her rank legitimate among her peers in other parts of the globe?

1) Next time your PC is at the temple, the infirm and elderly no longer adress him/her by their old title. They start using the new title. It is their preception that he/she has been promoted, and thus it so.
2) Popular perception of the followers.
