I am currently DMing in RotRL (we're about to get to Magnimar in the 2nd adventure). When my players prepared their characters, some of them were kind enough to write down elaborate backgrounds. A couple of those included in their story that the character had stumbled upon a strange item that could very much be much more than they looked, and I had planned on using such items as a kind of minor legacy item, if only to thank them for their background.
However, I have no experience whatsoever in designing such items, especially in "balancing" them so the PCs don't suddenly overpower everything in their path, hence this little call for help.
I have to "create" a kind of brooch, aligned to Desna and primarily a bane against undead, and a mundane-looking spellbook that is more than it seems.
For the brooch, I had thought that it could grant Improved Turning (as the feat) and that a natural 20 on the turning roll would automatically destroy the undead affected by such turning, but I was somewhat short after that. Extra Turning? Prot. vs. Evil?
For the book, I thought it could turn out to be some kind of book of Infinite spells, and/or create random scrolls every once in a while. Again, unfortunately, nothing more precise.

The brooch of Desna could be defensive in nature, rather than offensive. You could say it protects against the Ex or Su abilities of Undead. That would be kind of cool. No paralysis, no mummy rot, no stench... it'd be very handy and powerful, but not in a game-balance-destroying way.
For the spellbook, you could give the character an extra spell in the book at each level or - I kind of like your idea. Once per day you can extract a single scroll from the book of up to the highest level spell you can cast and which is in the book. If the scroll leaves your possession it instantly falls to pieces (to stop this from being something that can be sold or given to others).

For inspiration about the brooch dedicated to Desna, check out the article on her in Pathfinder #2. There is an item called the Monarch Talisman that provides protection from the elements and other, travel-related gifts. The description mentions that there are other versions of these talismans around and can probably provide you with some inspiration.
I would say that later powers should provide protection against traditional undead attacks: disease, paralysis, energy drain, death effects, etc. It might also allow the wearer to channel positive energy against undead.
As for the book, what about a spell book that grants the benefits of one or more metamagic feats--perhaps because of the way the spells are written, or something? As the character advances, study could unlock access to more feats. Alternatively, it could grant the equivalent of the spell mastery or spellcasting prodigy feats when memorizing spells from it. Or you could just use it as a vehicle for introducing interesting spells (from non-PHB sources, possibly) that just appear in the book, as though written by some unseen...and seemingly benificent hand.
If you go the route of having the spellbook produce scrolls, make sure that it is clear that the scroll only works when cast from the book. Otherwise, the book risks becoming a source of "infinite income."
