Starting a new RotRL game, with a new group, to playtest the PFRPG, and I plan to use the Medium advancement XP rules.
What this means is that the party, if they hit 90% of the encounters previous, will turn 6th level immediately before the fight with Xanesha. This will leave them a full level behind on Hook Mountain Massacre, and even assuming a successful fight with the Lamia, a TPK seems inevitable at a full level below par.
So, what I'm looking for is a good module to slip in either at the end of Burnt Offerings or Skinsaw Murders; Carnival of Tears is supposed to be good, and seems like it can be localized with the Whisperwood near Sandpoint, but I imagine Skinsaw's serial murders will be diminished in relation to the mass-slaughter of Carnival. Obviously, a GameMastery module will be easier to slip into Rise of the Runelords, but any good, appropriate level (4-6) adventure I can buy here in the Paizo store would be appreciated.

I can't cite it, but Joey is correct that "Fast" is the 3.5 default essentially, and has been mentioned by Paizo as the rate for the Adventure Path.
You're hardly required to follow that of course. Some definitely would like to add in extra adventures to space things out, and using Medium allows 50% more encounters of the same CR to keep pace with leveling. Slow allows 125% more (50% more than Medium).
However if you're not *wanting* to include another adventure, probably best to just follow the Fast rate.
Following the Medium (after the first few levels) will put a party about 1 level behind Fast, give the same encounters and xp earned. Slow puts them about 2 levels behind.

If you where in Hook Mountain already I would say you could use most of Bloodsworn Vale but change it around so that it fits in with HMH better. As it is nothing really jumps out as a sice trek. Have your players left any dangling loose ends?
Also have you looked at switching Xanesha with her counterpart from Hook Mountain. It makes for a less difficult fight.

I am running RotRLs, using slow advancement.
I am running D0, Hollow's Last Hope, right after the first raid. But instead of a well, the sickness is in the river, originally caused by a goblin corpse in the water. They get sent on the quest for the cure stuff from there. I am using the "Wishing Well" site for the dwarven abbey (Dwarves just used the site, didn't build the orignal building).
I did two setups during that raid, before the raid, wolf attack (werewolf from D1), as well as the chars rescued the kids from D1, who were in front of the White Deer Inn, for the Dog, Die, Dog fight.
Boar Hunt with Aldren - fight a werewolf (from above, and lead in to Jeva)
Dinner with Aldren - Monster in the Closet, after the dinner
Glassworks
Catacombs
D1 Crown of the Kobold King
Thistletop
This should put your chars in a good position for the end of the 1st module, probably ready for the end of the second one.

They're both correct.
Alpha 2, Page 104 under conversion.
"Existing adventures assume the fast experience point progression."
Then they tell you to add or reduce the number of encounters and difficulty if you run a different track using exiting adventures.
