If you check the "Start my month-to-month subscription" box, you'll be able to set up an ongoing subscription that automatically continues after your conversion credit runs out. This also qualifies you as a Pathfinder charter subscriber, and you will receive a FREE print edition of the Player's Guide to Rise of the Runelords along with Pathfinder #1.
I did make the selection to "Start my month-to-month subscription", but I'm not labeled as a Pathfinder charter subscriber, nor did I get the free player's guide. Why is that?

To get the charter subscriber tag, you had to sign up for a subscription (not just convert a magazine subscription over) before a certain date, which I believe was earlier in the month just before Gen Con. I'm not 100% sure how it worked, though... if you feel you should be getting the charter subscriber bonuses, I'd recommend you contact our customer service department or post a message over in the customer service thread.
