Without prospecting through the rules, I'll say that, like other such effects, multiple instances overlap rather than stack.

Pinned
Held immobile (but not helpless) in a grapple.

So, no coup de grace.

1- No, they do not stack. Imagine a lvl 15 wizard being taken down by 10 pesants with tanglefoot bags - shudder.
2- No, not helpless. However with the loss of its Dex bonus the creature pinned is subject to all sorts of fun affects like sneak attack.
