I'm hoping the conversion notes will be out by the time my online game reaches this chapter, but while reading through it, I did make some preliminary plans. Working from the world map in the front of the ECS, this is the approximate route I'm thinking of:
For individual encounters, I'm thinking of the following:
A - Fort Blackwell This occurs on the southwest coast of the island Sasserine is located on (I'm calling it Amedio Island, for convenience), the last point of civilisation before the ships cross the main shipping lanes to hug the coast of Q'barra.
B - Hungry Flotsam The Havekihu River is one of several that emerge from the jungles of Q'barra, and is the first that the expedition reaches after re-sighting dry land.
C - Tamoachaon Q'barra holds many secrets, some of its ruins dating back as far as the Dhakaani Empire, some far older. Tamoachan lies close to its southeastern shores, and the vessels reach it shortly before rounding the southern coast to head west.
D - Brotherhood Blockade The Scarlet Brotherhood are attempting to interdict trade to Adderport and Newthrone for reasons not directly related to the campaign. They have established bases on the two islands just south of Pitchwall, and from there their ships range out across Adder Bay. Though the Sea Wyvern and Blue Nixie attempt to give them a wide berth by cutting straight across to Valenar's coast, a vessel ranging southwards intercepts one or both of them in deep water.
E - Fort Greenrock This is, or was, a defensive and trading fort on Valenar's coast, due east of Taer Valaestas. It was wiped out by a tribe of near-feral lizardfolk, outcasts from Q'barra, who live in the jungles to the south.
F - River Menace At Valenar's southern tip, west of the long inlet that leads to Taer Valaestas, a smaller river flows out from the Jungle of Scimitars. Here, the ships stop to pick up water before making the crossing to Aerenal.
G - Renkrue The Aereni elves don't particularly like visitors, and the expedition avoid interacting with them. However, the island of Thal Taluna, just off Aerenal's southwest coast, is host to a small tribe of humans, descended from Sarlonan refugees who settled there around 1,500 years ago.
H-M These encounters take place during the journey south across the Thunder Sea, in much the same order as described in the adventure. The Isle of Dread is located just north of Xen'drik's easternmost reaches.

I have considered Kapaerian Island, however what has held me back is the somewhat close proximity to civilization. But I wouldn't know where else to put it with out modifying the geography of the world.
I think it's reasonable enough to add one extra island to the world, and it's probably better for a place as unique as the Isle of Dread to be a new location than to potentially tie it down to any baggage that may be related to an existing location. It's not as though Xen'drik's coast is thoroughly explored, after all.

Darn. I just realised why the version of Aerenal on the world map didn't seem to be a good match for the close-up version on page 217 of the ECS: That map is turned on its side, with north being right and west being up.
That means the path I suggested doesn't take the expedition past Thal Taluna, so it's not an appropriate location for encounter G at Renkrue village.
Possible modifications include putting Renkrue on the mainland of Aerenal, just west of the Jungle of Daggers, perhaps populated by 'back to nature' elves who've given up most of the trappings of civilisation, or else placing it on the Valenar coast just before the voyage turns south. Neither solution is quite ideal.
