I have seen the downloads for issue 97 (Life's Bazaar), and 116-137, but what about the issues in between?
Checking out my back issues, I've seen references to web extras for issue 100 (Githyanki scenario), and the monk Order of Man-Beasts is referenced in the Critical Threat for Lone Tooth.
Are there older extras that I missed, and are these in an archive somewhere, or lost forever?

Most of them don't exist. All the web enhancements that do exist are here.
In addition to the ones you've noticed, there is a 7-page enhancement for Dungeon #108's Challenge of Champions article, as well as enhancements for Dungeon #98, #114 and #115.
