I currently have 3 players and I am thinking of having them be gestalted characters has anyone started a campagin with gestalted characters?Should I make the major npc gestalted or leave them as is?
Yes, I just started, with 4 gestalted PC's and a fifth on occasion. I also started them at 2nd level with zero experience. For this chapter I'm not altering the NPC's at all. Really it is just a trial run to see how they fair. I would like to gestalt the major bad guys, but see it as a ton of work. I'm going to see how Life's Bazaar goes before deciding (if I don't gestalt, I might just add a level here or there to some main bosses.)
With only 3 players, I would not alter the campaign any.
If a new player comes in should they be made gestalted as well?
I would surely let them, as it wouldn't be really fair otherwise.
Is there a combination that should not be allowed?
Not that I'm aware of, so long as alignment restrictions don't come into play.
As for general thoughts on Gestalt so far:
1) Make sure to remember to reduce the CR's appropriately when it comes to XP time.
2) I like the fact that the party does not stop to rest as often. I'm amazed at how little they have stopped to rest so far in Jzadirune.

I have four players and have had them gestalt with the NPC aristocrat class with good success. Gestalting with aristocrat has had the effect of shoring up each character's weaknesses rather than granting additional strength. Plus by assigning each PC to an NPC citizen or family already defined in the campaign has really integrated the characters to Cauldron in very rewarding ways.
I'm in the middle of the third adventure and they are running at one level above the expected party level for the adventure, and coming close to two levels. I'm currently thinking about calculating their experience points as if they were a level higher than they are.
Gestalt characters are supposedly about as powerful as normal characters that are two levels higher. I'd recommend calculating all XPs as though the opponents are 2 CRs lower than actual. (If you do it the other way, calculating as if the PCs are 2 levels higher, you end up giving way more XPs, particularly once you start increasing the power of the opposition.)

If you make the PCs gestalted then the major NPC characters should be gestalted, also.
