So, how are warforged affected by the worms? They don't (as far as I know) have an organ which could be called a brain, and they're not affected by diseases anyway... Can a worm survive within them?

As I'm not trying to be biased here (I am playing a warforged in an AoW campaign) I see it as the worm gets to crawl around in the 'forged, dealing 1 point of damage each round, until either remove curse or remove disease is cast, or an appropriate Craft check is made (DC 20, following the rules for Heal checks on Kyuss worms), or the warforged drops to -1 hp and becomes inert. At this point, the animating force the worm was feeding on goes away and the worm dies.

Does that sound right?

Hmm, while I personally am still straddling the "disease/not disease" fence, I have to say that given the worms' capability to deal Intelligence damage without exactly resorting to poison (and potentially not following disease mechanics), a warforged would not automatically be immune to their mind-destroying attack.

After all, recent errata has noted that warforged heal ability damage naturally (such as that incurred from a Swashbuckler/Rogue's Str-damaging attacks, or a wounding blade), so I would say even your warforged might be susceptible.

I'm not sure if he'd become spawn (DM intereprtation based on the restrictions given in the template) but he certainly could be rendered completely "brain-drained" and potentially even dead (since they probably would feast till -10).

Just my opinion though- It seems like no one's supposed to be safe from these buggers, though your cold iron DR might do the trick...

Because I'm also the main DM for my group (though I haven't been able to DM in a while). Plus, just reading Dragon tells me that Spawn of Kyuss are involved... Ecology of the Spawn of Kyuss anyone? I also make an effort to play my character correctly regardless of any outside information I may have. AKA I don't metagame (well, other than the unavoidable) Also, my DM has never run a game before and isn't as familiar with the rules. So, I try to be up on what's happening so that I can better help her run the game.
