So my first group, the one furthest along, has just finished the Test of the Smoking Eye with heavy casualties, but not so much that they can't move on. After finally all reuniting on the material plane two characters short (and one resurrected), they are to set out on a new adventure.
But, with the party ECL ranges now 9-13, I find that they aren't quite high enough to move on to the next module. So I figure to run them through a heavily modified Fiendish Foot, and probably bring back Nabthatoron, who escaped when they faced him prior.
They've also made a few more enemies who survived the encounters somehow, notably Zaur Sza the rakshasa, and Myrkul the Lich (who was killed, but as his phylactery wasn't anywhere nearby...).
What have other people done with bringing in enemies with vengeance on their mind that have escaped from their PCs in the past?

I'm about to run the last leg of TOTSE and my Players will be a bit behind too; I'm not going to use any surviving enemies in the encounter (mostly because i'm not sure if any of them will live or not) but am taking advantge of the fact that Plain shift can drop them between 5 and 500 miles from where they want to be... and i will be adding (thanks to the sugestion of someone on this boad... sorry don't remeber who) to use part of "The Black egg" from Dungeon 106
dohort did not live threw the kou-toan Fortress in Senith trajetory (he should have but i made a mistake as a DM and told them how many HP he had as they had one more attack as he was running away) However i have desited that they halfdragons are all half brack dragons and are insted of trying to release an army of findish Dragons this "black egg" will beable to bring back the Dragond father as a older greater Dragon... (expecting two age catigories) with the only probelm being he comes back with 0 HP...
I don't plan to acctuly let the half dragons bring him back be it will be the plain... I also had fun building advanced half dragon Chokers they are only CR 8 (I think) but they should be fun.. also half dragond kobolds that are workers for the black orcs who will only use their breath weapon and run...

Yeah, my group never actually ran into Darlot the dragon-father. The Erinyes killed all but one of the characters, who was told to run away--he ran into Zenith, took him out alone, and left the keep...so I suppose he could be yet another potential enemy.
As for the Fiendish Foot, I'm probably going to alter the elves to be a group of advanced kobolds, because I'm a fan of them, and they like to show up all through a campaign for me, even if it's a throwaway line ("About halfway there, a group of kobolds attacks you, and you make quick work of them.") Not sure what I'm going to do about the groups outside the tower though. I'm still trying to make changes.

My group never ran into Dhorlot either. They retreated with Zenith as soon as they found him. Dhorlot will probably be brought back if they ever decide to venture to the north again. Skaven also survived the encounter in Flood Season, and after killing the party cleric, my group wants their revenge. I am replacing Fetor Abradius (Soul Pillars) with Skaven.
I've tried to foreshadow most of the villains in the AP. Some of the 13 for example, have terrorized the PCs in their backstories. After retreating with Zenith, the party was besieged by a group of ogres led by Zarn Kyuss who slew the barbarian. They have just begun the Demonskar legacy.
