I am currently running Zenith Trajectory and, unless I overlooked it, there is no mention of why Zenith went insane (other than losing all of his comrads) or the source of his prophecies. From the background of the story, it appears to be that his prophecies involving the Huo Toa were correct. Later, when you encounter him, they are random. Does he have a prophetic power, and if so, what is the cause?

I had several interpretations:
- Zenith is supernaturally marked by the Carcerian Sign; it could naturally introduce insanity over time (which is why the young boy from the first module is yet to be affected);- His inherited family instability, the utter destruction of his crusade (and his being the last survivor) and a crushing loss of faith during the defeat lead him to have wild visions, possibly sensed by Blibdoolpoolp near her shrine - which leads her to project upon him some prophesies (to suit her will, of course);
- Blibdoolpoolp sees potential in this strong leader and claims his quickly crumbling faith for herself, revealing herself to his mind and taking him into her church.
You're welcome to build a composite of all of these traits and circumstances from these points.
It's plain that Zenith became a prophet, based on the accuracy of some of his visions; whether it's the lucky ravings of a madman or the sight of an oracle is up to you as a DM to impart on your players.

My vote = Zenith got hit with some of Adimarchus' disembodied dreams of madness and went bonkers as a result.
And instead of claiming his visions come from the Eye in the Darkness, I'd have him claim his visions come from the Smoking Eye. Just a handy way to forshadow Alek's similar insanity and the whole thing on Occipitus.

Is that a bit of change that we can expect to see in the hardback?

As a matter of fact, it probably is.
