My girlfriend got me a subscription as a gift but she doesn't want to use her credit card online and I lost the card that came with my first issue.Should we just mail a check with a new subscription card to let the subscription department know what account it goes to or just send the check with a note explaining whose subscription this is paying for?

Just send a check with a note or a sub card filled out. As long as the address matches, we should be able to match the new sub up to your old one. It definitely helps if the note on the check has the name of the intended recipient and the name of the magazine.
