Does this mean the attack does 1d4+10 wisdom damage?

If a witch hex doesn't work (due to target making the save or being immune or whatever), does the witch realise?

I'm reading the stat block for the Dracolisk and it says that it has a breath weapon and gaze attack.

Special Attacks breath weapon (60-foot line of acid, 7d6 acid damage, Reflex DC 18 half), gaze (DC 18, see Basilisk)

How often can these be used? The general description for breath weapons says that it should list a frequency but this is missing from the Dracolisk.
