My thoughts were that Kesnet’s mother lives in a bordello, and Qesnef was a customer while magically altered. Kesnet’s mother loved her ugly baby, so the young lad grew up with a dozen aunts, and was useful to have around - first as a hardworking and surprisingly honest scullery labourer and then as he…filled out/up, bouncer/muscle. A short stint with a mercenary company (I pretty much imagined something eerily similar to Archpaladin Zousha’s “hiring vignette”) in pay of the Sea Princes helped him learn weapons and a little seafaring. He is on furlough and probably smashed some unwelcome heads to save a poor helpless soul in view of the plot’s employer.
So in the dodgy part of town, Kesnet is a minor celebrity/known figure/local identity. He’s friendly, can obviously handle himself and doesn’t treat with fools gladly. Elsewhere is Sasserine, he’s pretty much persona non-grata.
I guess he might also have a theme of “Ghetto Superstar”, with “Ear to the ground”, “Favours Owed” as Power tags, with a negative of “Double Aaaaaagh!?!?!” that occurs out of the ghetto, but I feel Annelise might have all that too…Depending on which quarter Annelise is from, they’ll likely know each other…
