Name: Muirios Megdalia
Class: Warpriest 17
Adventure: Kings of the Rift
Location: The Vault!
Catalyst: Big dragons do big damage.

Muirios, heroic warpriest of the drunken god, charged valiantly into battle against Xyzanth and Brazemaal. Unfortunately, Xyzanth crit the bite, greater fort did nothing for the crit, and Muirios failed his save vs the con drain. 12 con drain later (hello high roller club!), Muirios was very much dead. Sometimes you just get unlucky and die.
It's cool though, he came back to life later. Poor guy. First death in 18 levels.

Name: Zarina
Class: Slayer 16
Death: Boneyard bone slurp.

Poor Zarina. Provoked an AoO while charging in for a "blaze of glory" style death against the Boneyard. She did not survive the AoO, and got her bones slurped.
Thankfully, she got better.

Alas, our first death in the Library of Last Resort.

Class: Arcanist (Occultist) 14
Death: Weird!
The party is slightly underleveled thanks to being 5 instead of 4.
Ievis unfortunately was weirded out by the nightmare tree and died instantly. He's back to life now though. Good old high level pathfinder.

Name: Zarina
Class: Slayer 10
Adventure: Gather of Winds
Location: The portal with the abyssal ghoul.
The abyssal ghoul landed every attack and rolled high on sneak attack before the slayer got to act. She was raised back to life and had her negative level restored, so all is well.
The players have pretty much reached the point where barring a TPK, death is just an inconvenience.

Names: Marnie and Lesley
Classes: Treesinger Druid and Dirge Bard 8
Adventure: Hall of Harsh Reflections
Location: Zyrxog's Lair.

Too many lightning bolts does a dead adventurer make. The surviving cast was able to force Zyrxog to flee. Perhaps someday he'll return to finish the party off, but for now, he's content to lick his wounds and plan for the future.

Class: Treesinger Druid 6
Adventure: Hall of Harsh Reflections
Location: Streets of Absalom
Catalyst: Poor choices

Even if you're a ball of claws, grappling a ball of claws is still a pretty bad idea.
Party was willing to fork over the cash to get her raised though, so it's not the end of the world.

Name: Cordelia
Class: Sorcerer 2
Adventure: Whispering Cairn
Location: Sunken Showers
Catalyst: Paralysis Underwater

When the group was fighting the small water elemental, the ghoul in the neighboring room decided to take advantage of the chaos and join the fight. Cordelia's friend Shinoko, the party's Oracle, was the first to become paralyzed. Cordelia would bravely give her life, putting herself between the ghoul and its meal, allowing the rest of the party to drag Shinoko through the water to safety. The ghoul, of course, was perfectly happy to trade one succulent humanoid for another. Cordelia was quickly bitten, paralyzed, infected, then next round coup de graced and eaten.

Cordelia will be missed, and now the party has both a grim reminder of their own fragile mortality and even more reason to finish exploring the Cairn - for their fallen friend.
