Had our first death last night when a certain ghost guarding a star stelae in book 5 rolled 2 nat 20s and near max damage on her force punch. The gnome didn't know what hit him as he was launched off the top of the building after failing the fort save in the surprise round of combat. After defeating her, the party managed to find the puddle that used to be their friend and used their only raise dead scroll on him (my party has no spellcasters, so that scroll was one of their only lifelines).
