Pg. 42 - Washing Away the Soul sidebar
In the second paragraph of this sidebar, the fourth sentence starts with:
Creatures immersed in one of the springs' pools are affected as if exposed to 2 doses of Lethe water.
I'm uncertain what actually is meant to happen here. Normally being affected by 2 doses of the same poison just resets the frequency and adds a +2 to the DC to resist and save against it (so the DC would be 18 here). It's unclear if that's all this was asking us to do, or if it meant for the afflicted to suffer double the effects on the first round after a failed save.

Pg. 46 - E3. The Holding Pens (CR 12)
In section E3, second paragraph, the third sentence says that one of the ghoul overseers augments the ghast minions with inspire courage and haste. Unfortunately, inspire courage wouldn't work here, as undead are immune to both mind-affecting effects and morale effects. Inspire courage gives both morale bonuses and competence bonuses, but it's all a moot point since it's a mind affecting effect.
A GM would have to look to an old 3.5 D&D feat from Libris Mortis that allowed bardic performances to affect undead.

I apparently had the rules wrong here. It adds +2 to the DC and then adds half-again as much frequency. So DC 18 and 1/round every 9 rounds. No double damage (presumably).
