Well, it's a personal spell, so it can't be a potion. So either scroll or wand. Wand's better if you can afford it.

I would add it to a Animated Portrait. While you can ask simple questions and get the standard magic mouth reply, you can also, if you know the right activation method, also use the item to contact the ancestor depicted.

I would rule a larger bonus, but a more restricted ability to be applied.

Either that, or the image morphs into the image of the ancestor who is providing the desired wisdom.
