I'm the event coordinator for a big event going on in a week, and I've noticed some stuff that would be nice to have on the default session sheets.
Now, I know it is too late for me to get these changes before that event but maybe this will help someone else or be around next time.
1) We need to have a spot to enter the scenario that was ran. In case we have an event that has more than one scenario available. I've seen GM's forget to write down what scenario they run at past events.
2) We need boxes for the GM's to check the possible victory conditions. I believe there are only 4 possible ones: A, B, C, and D. Or, we could just have a line to enter them in.
3) Do we still need Shadow Lodge and Lantern Lodge? Those could be removed.
4) We might want to have a '-' already entered or a separate entry for PFS# and character#. I've had too many people leave the character number off.
I am thinking of making some changes to my download for my event with those in and photo copying those changes.

One of our local game stores (two, possibly, if I remember) has their own sheet for reporting information. It has a spot to put the scenario run and the next batch, I've been told, will have the victory condition boxes. Until then, GM's have been instructed to simply just write them down.
As for a - for the PFS#, looking at a sheet now it says "Character #". If people aren't putting the full character number (PFS player number included), then that's user error on their part.
Huh, also looking at it, it seems Shadow Lodge and Lantern Lodge are no longer options. Also, the name of the scenario is right there at the top. So now I'm just confused as to where this is coming from. Must look at other scenarios' sheets.
For reference, the one I'm looking at is #5-09 - The Traitor's Lodge.
Out of curiosity, are you looking at older scenario tracking sheets which don't need victory condition boxes A thru D and which still had the two erstwhile factions?

Also, Sior, the generic ones created by the website don't have room for the scenario, IIRC.
However, for the reporting sheet, you are fully authorized and allowed to create your own, as long as it includes the information your coordinator will need to properly report the table.
It can even be taken on a sheet of blank notepad paper.
Really, the sign-in sheet is purely for the local GM and event coordinator's use, and becomes a throw-away once the players get their chronicles, and the event has been reported on the Paizo site.

Sior, I am talking about the one Paizo gives you on the Event Coordinator page where you report it. Too many GMs don't print out a reporting sheet and those have the Event number already filled out.
Also, if someone doesn't include their character number with their PFS number, it can be a PAIN, to track them down to find their character number. I'd rather give them a reminder. Especially since they usually write down their character name not their name. How do I know who Revan Ravenloft is if the GM and I don't know the person? Especially if he is from out of state? I'd give a little help to reduce the number of times this occurs by putting in a visual reminder that there are two parts to the character number. I still have sessions from over a year ago that have not been fully reported and the people did not respond to my email so there is nothing I can do about it.

The original post is slightly unclear.
Terek is making suggestions for the session sheets the website will create from within the event reporter, not the session sheets that come with the scenarios themselves. These are the sheets that come auto-populated with event name and number.

This may be coming out of left field, but what exactly is done with the sign up/session sheets? Are they mailed to paizo's campaign manager? Or are they just temporary until the data from the sheet can be reported online to this site?
Completely for the GM/event coordinator's use. After the results are input into the reporting system on Paizo.com most of them are balled up and trashed. (Though I suggest holding on to them for a bit in case a player has an issue.)
