So my question is: How exactly does one award a player who demonstrates excellent role-playing or team-work, a Vanity?
And how large a Vanity may I award? 1-3PP vanities for low-levels 3-5PP mid level, etc...?
Would I write the assigned Vanity on the Players' Cert, and then initial it as GM?
Has any of this been addressed in a FAQ?

Again, I don't think that applies to PFS play. You can't even reward equipment, how can you reward vanities? That information is for people using the Field Guide in their home games.
Are you sure? It is the Pathfinder Society Field Guide, so you can't get much more PFS than that. Are you saying it's for home campaigns that use PFS?
Then those Vanities would not be valid if they took there characters from the home game to a con... If I'm reading your response correctly?

This doesn't apply for PFS sanctioned play. Keep in mind, the book is not just written for sanctioned play, but also home play such as with Shattered Star AP. In sanctioned PFS play, a GM may not reward any free vanities. They must be purchased with PP.
Ah, and there lies the distinction.
