D31 just has the reinforced door. The Chaos Beast in D32 will come up to that door and possibly attack through it if it hears anyone approaching, which is what the illustration on page 22 depicts :)

It should of probably been notated as D31a: Door & D31b: Room/monster/etc.
That was certainly an option... but I decided not to do that since the door and the room itself are pretty far away. (Also, I'm not a very big fan of using such long map tags, and avoid going into the lower case letters when I can avoid it.)
In any event, it's not an error.

I can understand and respect that decision. I remember as I was writing that out, it felt clunky being so long in notation, but in my sleep deprived status I couldn't think of a better, less confusing option.
