Hey all! I was hoping to get some ideas for a magic item centered specifically around a Wizard. The Wizard has the Dream touched feat from the SCAP hardcover book, so I have been foreshadowing events and such through his dreams, which I really enjoy. Well the other night we had a session in which for some stupid reason a mentioned that he was in a dark room with a table. On the table was a leather bound book with an engraving of a person screaming. Of course he opened it up and I had him make a Will save (I'm not quite sure why..."), and I told him that his character began to clutch at his head as sounds of thousands of angry voices began to echo in his ears, all screams.
Now while I was ad libing the idea on the fly, I didn't really have anything to lend weight to what the effects might be if he failed his save throw (thankfully he rolled a Nat 20 on that one cuz I would have been lost for a few if he had failed). So anyways, to make a long story short, I kinda like the idea of maybe a madness effect that could be a side effect of this book, but I would like to either tie it into the storyline as a plot thread or a magic item somehow. I have searched for some decent "book" magic items and haven't come up with anything overly useful or interesting in my opinion. So I was hoping for some ideas if anyone has some. Thanks so much in advance.

That'd be quite simple enough to do. :)
During the encounter with the mummy lord in the Cathedral of Feathers, simply add in a thick book on the table before the mummy. If the PC's reach for the book, the mummy will simply shake his head in warnind.
'Fools. Mortal minds cannot comprehend the power that resides within this book.'
That's when the PC who opens the book discovers that it has been trapped with your thousand voices screaming trick.
As for what the book contains, how about simply making it a diary of Adimarchus from before the fall of what once was Occipitus from Celestial. That would give them some ideas of what he was like before the great fall, and his slide into evil.

What he saved against was a variant of the phantasmal killer spell.
The dream touched book can have some prophecy. Things like what zenith and the paladin were spouting while crazy. Give him prophetic dreams of future scenes of the adventure path when he sleeps. Eventually, he will be spouting insanity just like those affected by the insanity. If he sees Vhalatru, have his character spontaneously attack him as if he was affected by confusion for a round. without understanding why.
As far as ability, while taking a standard action to brandish the book, anyone who sees it can be affected by confusion. Treat it as the Umber Hulk's gaze attack.
Once a day, he benefits moment of prescience as an immediate action with an instantaneous duration. If he uses this ability, he must make a will save or be confused for one round.

It would depend on how quickly you wish to introduce the book.
The obvious answer is to link the book to Adimarchus and/or the Cagewrights.
The book could contain the history of the organisation, the detailed ritual that could open a planar gate to Canceri and possibly some madness spells such as confusion, Power Word, Feeblemind etc. Another power of the book could be that the face on the cover can talk to the owner and if they can resist its maddening gibberish they can be linked dirstly to Adimarchus. Through this they maybe able to ask a question, or learn more of his past etc.
The Save could be something like DC30. Franks idea is also good when the book is first opened.
This book could be found in possession of Fetor (Ch 7)or Shebeleth(Ch 10).
