Hmmm, what's the party level? Are they ready for direct divine intervention? I'm assuming you want to avoid predictable ideas like "crossing through an ill-defined border between planes" or "being 'Summoned' by a powerful, extra-planar mage". A great device that I have yet to borrow from Michael Moorcock's Elric novels is as follows: "Out of a fog bank silently floats a vessel of strange and ancient design. Over land she sails as well as sea, this dragon-prowed galley from the depths of Time. A figure raises a hand in greeting and gestures the party aboard. Upon the deck stand two figures, twin brothers. The captain stands upon the quarter deck and greets you; his eyes are cloudy and staring for his is stone-blind. His identical twin brother, the first mate, is tongueless. The captain knows the party by name and tells them that he has come to take them to their Destiny, although he does not know what they should expect. Below-decks the party encounters other adventurers who have been picked up and are being ferried throughout the multi-verse to their own Destinies. Rumors abound as to who the brothers are and where they come from. All that is certain is that they have sailed aboard this strange ship for time out of mind and have the respect, and even the protection, of a variety of divinities. The ship sails through a fog-bound ocean, stopping every once in a while to pick up or drop off passengers until finally the party is called on deck. Athwart the starboard side a jetty emerges from the fog; high above the party spies sky-high towers and floating airships. "Step lively my lads," the captain cries, "and ashore with ye. You have come to meet your Destiny, for better or ill. Step ashore onto EBARRON!"

I transferred my party from the Forgotten Realms to Eberron, using the portal in Shraevyn's Tomb northeast of Shadowdale (The Sword of the Dales-Trilogy; originally this Portal lead to Sigil). The Problem is that in principle,I'm mixing two different wheels of planes, but my thought was, if Moorcock could develop a multiverse, than I can do the same with different D&D-universes.
But I admit the ship may be the better (and more purist^^) solution.
