I obviously can't speak for others, but I use PhotoShop. I know from hero339's DeviantArt page that he uses PhotoShop too.

most of the stuff I use comes from the dundjinni or cartographers guild forums, occasionally I build things from scratch (but so far haven't done for that AP)
