Tips on incorporating character backstory

Hello!

I'm running this game, 8 years after it was released, and it is great fun! My group is just about done with book 1 and, especially given that Book 2 is a bit more open ended, I was hoping to start working in some backstories of characters.

One in particular is playing a dreamspun sorceress who in character permanently suffers nightmares. Having read ahead on some plot mechanics and hooks I know this ties in pretty nicely with the stuff of Carcosa, Hastur, and Xhamen-Dor. But I'm still trying to figure out exactly what I'd like to do with it!
Getting to Lvl 20 after Strange Aeons

I've actually been playing around with expanding the plot beyond the end to have them encounter a full fledged old god

End of campaign Spoiler:
Xhamen-Dor possibly? You fight it's husk, and then a star seed, so a semi-dead one and a nascent one, but never the full thing. Was talkijng with a fellow DM about the potential reasons and I've kind of settled on the stars just being right for Hastur to exert more influence on Golarion and force an NPC ally who had knowledge of Xhamen-Dor to "sprout" , to that end could have them even dip into mythic a little bit to deal with such a threat
