If due to exposure to magical or environmental effects you were born with an unusual eye or hair color that is outside the normal range for a human, would you be unable to use a second dose to return to your previous appearance after using the first one to aid in a disguise?

If two PCs are identical twins and one got a tan from a sunny vacation and the other could not join in, would the paler twin be able to match the tan with a serum of appearance change? Would this violate the restriction against taking on the exact appearance of another creature.

Quick shield block gives you a reaction that must be used to shield block. It has a prerequisite of Reactive Shield, so it seems intended to allow you to shield block even if you used a reaction to raise your shield per Reactive Shield. If you spend an action to raise your shield does this mean you can now shield block twice? Would adding Shield Paragon (FTR 12) allow you to shield block twice while you were in the stance?

Sure this might be hard on your shield, but considering that as an option for a character.

Is Righteous Might intended to give an AC penalty to offset all of the bonuses it grants?

It grants you an AC of 27 when cast at 6th level. A cleric capable of casting a 6th level spell is 11th level. If you are wearing breastplate or chain mail and have a dex of 16 or gained heavy armor proficiency and have a dex of 14 then you would have an AC of 28 (11 + 7 + 10) in mundane armor without a shield. This flat AC would also mean you lose more AC from casting this spell as you go from level 11 though level 14 until you can cast the level 8 version of the spell.

The AC of the level 8 version is one point higher than what someone with mundane armor and no shield could have at level 15.
