Is still the RAW reading of the rules "Dodge bonuses stack with other dodge bonuses even if they are from the same source" still correct with the exception of "offensive defense"?

So if a PG with outslug style makes two 5-foot steps (with an advancing weapon or in enother legal way) does she gets two times the dodge bonus?

Am I missing any rule update?

Is it possible to release the grapple in the first grappling round?
If it is possible, is the following example correct?

Ex:
There is a monster with 4 tentacle attack each one with grab and constrict.
It is the monster turn.
- The monster attack with a tentacle and succeed.
- He starts a grapple as a free action (so he does not take the -20 penality for grappling only with the tentacle) and succeed.
- He does constrict dammage.
- As a free action he breaks the grapple.
- He does the same with the other 3 tentacle attacks.
