in between sessions, we are resting outside of a dungeon. We all level from 2 to 3, roll our new health, add on feats + skills, etc.
If I gain 5 health and go from 15 health to 20 health, so I automatically go up to 20 health, or do I start at 15 and have to heal off that extra 5 with resting/potions etc?

It would be silly if you were healthy, and leveled up, and suddenly became injured.
Well, it's pretty silly that a gnome can charge around a dungeon on a dog with a kiddy-lance and still do about as much damage as a half-orc charging on a heavy war-house, but it's the rules.
It's pretty silly that a gunslinger can reload the a double-barelled gun in each hand and shot a thousand times in one 6-second round, but it's allowed.

By health, I assume you mean hit points (hp) right?
Yes.
You add additional hit points to your total, maximum hit points each time you gain a level.
I understand.
Whether or not you happen to be at your maximum hit points when you gain the level and get your additional hit points does not factor in.
This sentence makes no sense to me. My question is: When you add to your maximum hit points from gaining a level, do you also add that number to your current hit points?

Hit points aren't really tracked by "current", but by "wounds". Your current hit point total is always equal to your maximum minus the wounds.
The wording in the system is a bit more awkward, but the general rule is: If your maximum HP changed, your current changes by the exact same amount in the same direction.
So if you leveled, you would have the same number of missing HP compared to your new maximum. This could result in you going from "dying" to positive hit points, but at least it means you won't ever go from full HP to hurt (which is harder to swallow, IMO).
