What is the creepiest critter you've raised with Create Undead?

I'm interested in going for the squick factor, but I'm running low on inspiration. I'm talking about tacking multiple creatures together, combining just a few too many appendages, or recreating the gibbering mouther via necromancy. I'm looking for a little unpleasant brainstorming from my fellow black mages here: What's the nastiest piece of work to ever shuffle off a slab in one of your games? How did you represent it mechanically?

Zombie T-rex that was animated right after it had pulled a Jurassic Park on an entire village of peasant so all the people corpses in its stomach got animated too. It hurled those zombies at us as unliving amunition.

I remember seeing Monte Cook do this mess back in his Dragon's Delve megadungeon. It was a ghoul worm that barfed back the people it ate as ghouls. Wish I still find the art... That thing was straight up freakish.

The most accomplished necromancer I ever had possessed the following:

Bloody Hill Giant Skeleton Bodyguards ×10
Crimson Worm Fast Plague Zombie
Juju Zombie Cyclops Servant
Sea Serpent Fast Plague Zombie
Roc Fast Plague Zombies x2 (mated pair)
Human Fast Plague Zombies x150

The hill giants carried longspears and surrounded her at all times while on the ground, so nothing got within 20 feet of her without getting pin-cushioned.

The cyclops served as a lieutenant while the rocs served as aerial steeds for my necromancer and her lieutenant.

The serpent and worm served as aquatic and subterranean transports, respectively. They were hollowed out so that the undead hoard could ride along inside their bulk, ready to be spewed out once their carrier had made landfall.

The worm in particular allowed my necromancer to discreetely sink hole an entire city before releasing an undead hoard on the survivors. Took weeks of work to pull off.

I once had a monster encounter that was an undead spider who could breathe flying spiders at people.

I liked it. My players didn't appreciate it as much :D

I'm getting some serious Castlevania Season 3 Forgemaster Isaac vibes here. And honestly, it's an angle I hadn't considered. I've been sitting here thinking about shooting for "one big freaky thing." Having a legion of the dead is another angle entirely. That's a straight-up menagerie!
