I'm still fairly new to 2e. How is a monster ability usable 1/day denoted? Like can you stick a x1 on it like an innate spell? Or does it need a Frequency and Effect block? If it has Frequency does it need to say Effect? Are Frequency and Effect only associated with actions, or could it be part of an automatic ability?

Archon's Door: Once per day, if an archon sees another creature cast dimension door, it can cast an innate dimension door (heightened to 5th level) within 1 round to attempt to follow that creature to the maximum distance of the archon’s dimension door. If the archon’s dimension door has enough distance, the archon appears the same distance and direction from the creature as before either used dimension door.

I'm sure we can help clear up things for you, but talking in the general and the abstract makes things appear much more complicated than they need to be...
It seems pretty obvious to me that they are talking about proper formatting for a homebrew monster.
Regarding your last question, RJGrady, I don't think I've seen a passive ability with a Frequency line, so I'd suggest including the limit in the ability description instead. Alternatively, make the ability a free action.
