<p>I agree that this item could be easily adapted to be a cursed item for animal companions though. perhaps instead of humanoid creatures only, it's animals and magical beasts only. Then familiars and animal companions are at risk but since their master is unaffected the companion ends up just looking neurotic.</p>This item seems too similar to what's described in the song Creepy Doll to be a coincidence. Using break enchantment or remove curse on the doll makes it unable to return for 1d4+1 days, though at that point, it returns as usual.</p>
<p>The doll is quick to forgive, however. If the owner treats it well for a day, carrying it openly and maintaining physical contact, the nightmares fade away until the doll feels that it has been mistreated again. The only certain way to get rid of or destroy the doll (short of miracle or wish) is to find someone who accepts it as a gift. If the owner dies, the curse is lifted, and the doll patiently waits until a new owner finds it, though if the previous owner returns from the dead before that time, the doll returns to his side.</p>

<p>So this is actually kind of a cool idea! The big drawback I see is that there are a bajillion possible sorts of familiar or companion, so it's really hard to tailor an item that would affect every one — after all, a snake, a wolf, a hawk and a harbinger archon may be interested in very different things.</p>
<p>So, maybe have the item shapeshift or adapt to be something appropriate. Non-familiars don't see anything there at all — it's just an invisible knot of magical potential, sort of like a haunt. When a familiar touches it, it condenses into an item appropriate to that familiar. The familiar must then make a Will save or the curse kicks in. (This seems like the sort of thing a jerk wizard might come up with for lulz, and leave hanging around the local Wizard's Academy.) It can be either crippling or merely annoying. I think I like the latter better — your familiar has to make a Will save to do anything, otherwise it's lost in contemplation of the item, or gnawing on the chew toy, or whatever.</p>
